In this article, you’ll learn what Mazzella Companies’ health and wellness benefits package has to offer and how much those offerings will cost you as a new team member including:

Who is eligible for health and wellness benefits?

All newly hired team members will be eligible for health and wellness benefits at Mazzella Companies on the first of the month following 30 days of employment.


What Dental benefits does Mazzella Companies offer?

Team members are offered three dental plan choices – Basic, Advantage, and Advantage Plus. All three plans work through the Aetna Dental Administrators network of providers. While the Advantage Plus plan covers $1,500 orthodontia coverage for the plan holder and their dependents, the Advantage plan covers 50% of major services such as denture work, crowns, onlays, and inlays, and all basic care including diagnostic x-rays, extractions, fillings, and emergency work is 80% covered by all three plans.

All three plans cover 100% of preventative and diagnostic care, with a limit of two treatments in a calendar year for either plan. Any visits outside of these regular visits will be subject to a $50 per person deductible.


What Vision benefits does Mazzella Companies offer?

Mazzella Companies offers one vision benefit plan through United Healthcare, using the Spectera Network of providers. When participants use in-network eye doctors, the plan will cover a comprehensive exam, standard scratch-resistant lenses once a year, and $130 towards the frame of your choice. Contacts that are necessary will be 100% covered when participants use an in-network eye doctor. There is a $15 copay for any frames purchased and a $15 copay for the exam.

Mazzella Companies also offers plan participants access to the Laser Vision Network of America (LVNA) to provide members with access to discounted laser vision correction providers. Members receive 15% off standard or 5% off promotional pricing at more than 550 network provider locations and even greater discounts through set pricing at LasikPlus locations.

In addition, Mazzella Companies offers up to 20% off on additional pairs of eyeglasses or contact lenses that exceed your benefits and are purchased through an in-network provider.

For those who need prescription protective eyewear, Mazzella Companies provides a list of retail stores where team members can purchase OSHA-compliant, corrective, safety eyewear at a discounted price. As long as they have a valid prescription, they can purchase and be reimbursed up to:

  • Single Vision- $100
  • Bifocal- $170
  • Trifocal- $200

What Medical benefits does Mazzella Companies offer?

Mazzella Companies offers two separate medical healthcare plans. A discount is available to non-smokers if they have not used tobacco products in the previous six-month period. Plan participants have the option to enroll in a traditional plan or health reimbursement arrangement plan. Any virtual doctor visits, or telehealth visits, will be considered a non-routine visit and any copay or deductible will be applied, depending on what plan you choose.

No matter what plan is chosen, routine well adult care exams, including screening tests, immunizations, and preventative care, and routine well childcare exams, including screening tests, preventative care, and immunizations, are all 100% covered by both plans and not subject to a deductible.

Referenced Based Pricing

Reference-based pricing, or RBP, is offered in every state except for those states where RBP plans are not widely accepted. Under RBP, team members partake in prices for services that are capped at 40%-80% above the Medicare rate.

CIGNA

The CIGNA Preferred Provider Organization (PPO) is the only network option available for those who live in a state that is not RBP-network friendly and a second network option for those who live in Ohio and North Carolina. The plan allows coverage for team members who are either not eligible for RBP or whose provider will not agree to RBP.

Evolutions Network

A network of providers available to Mazzella Companies team members who live or work in Florida. This network allows coverage for team members who are either not eligible for RBP or whose provider will not agree to RBP.

Traditional Plan

The traditional plan is offered to all team members in the United States, whether they are using an RBP, CIGNA, or Evolutions network. Copayments for individual services provided range from $10 for a convenience care visit to $250 for a non-emergency ER visit.

Health Savings Account (HSA) Plan

Through the RBP, Evolutions, or CIGNA networks, Mazzella Companies offers a Health Savings Account plan (HSA). An HSA is a high-deductible health plan, the member is responsible for all eligible services up to the listed deductible amount. Once a member has met the deductible, the Plan will pay at 100% with the exception of Prescription Copayments. Once you have met your Out-of-Pocket, the Plan will cover your prescriptions at 100%. Preventative services are covered 100% before the deductible is met.

“The HSA plan is a health savings account,” Emily Cavazos, Mazzella Companies’ Total Rewards Manager, says, “and it has the higher deductible.” While the deductible may be higher, the cost of the premium is less, amounting to less money out of your paycheck, and Mazzella Companies contributes $1,000 for self-only coverage and $2,000 for family coverage on an annual basis – this employer contribution does not count as taxable income.

The Health Savings Account is an account funded to help you save for future medical expenses. This is your account; the money is yours and it will stay with you if you ever part ways with the organization. The money in it will roll over year after year and will never expire. You are responsible for managing this account. You will receive a debit card to access your funds, and there is a mobile app to help you manage your funds.

Flexible Spending Account

A flexible spending account (FSA) is a special account team members can set aside money in to pay for certain out-of-pocket health care costs. This money is not taxed and will be pre-loaded for the plan year onto a Debit Mastercard that can be used on qualifying expenses at the point of sale.

Plan participants are eligible to rollover up to $640 of unused contributions for the next plan year, and so any unused contributions that rollover from your prior year plan will also be automatically loaded onto this card for the year ahead. The rollover amount is available even if no contributions are made the year that follows the balance year. Currently, in 2024, Mazzella Companies’ team members can contribute up to $3,200 to their FSA.

Dependent Care Flexible Spending Account

A separate flexible spending account is offered expressly to set aside money to pay for dependent care. The plan will reimburse team members for expenses related to dependent care for dependents under the age of 13 who live with the plan participant more than 50% of the time and attend a care facility so the plan participant and any spouse can continue to work during the day. Participants who have this plan will be able to pay dependent care expenses while also lowering their taxable income.

Teladoc

Teladoc is a no-cost added benefit to the healthcare already offered at Mazzella Companies. They offer 24/7 general medical and pediatric care when a team member can’t make it to the doctor’s office or doesn’t have a general physician. The physicians in the Teladoc network can also prescribe treatment for non-emergency conditions like the flu, allergies, bronchitis, and more. Teladoc also offers sexual health lab orders, without ever having to visit a doctor’s office.

Virtual Doctor Visit

Tobacco Cessation

Added in 2021 and available at no extra cost to team members and their families who are enrolled in Mazzella Companies’ medical plan, the Tobacco Cessation program is provided through Teladoc, with a hotline available 24/7/365.

Emily notes, “It’s ongoing support at intervals, and they will walk you through the resources. ” She continues, “Because these are doctors, they can prescribe any needed medications.” After six months of tobacco cessation, plan participants will see an approximate 37% reduction in medical premium costs.  

Short and Long-Term Disability

Mazzella Companies offers their team members short and long-term disability insurance as income protection when they are unable to work due to illness or injury.

Short-term disability is an income replacement and covers off-the-job accidents and illness that workers’ compensation would not cover. The benefits begin on day eight of the plan participants illness or injury and covers 60% of a team member’s salary, to a maximum of $1,500 per week, for up to 11 weeks.

Long-term disability works in tandem with short-term disability to provide income for long-term illnesses and injuries. Once short-term disability benefits are exhausted at day 91, long-term disability will begin covering 60% of salary to a maximum of $5,000 per month and can be paid until the normal retirement age of 65.

Life and Death Insurance

Mazzella Companies offers team members basic life insurance and accidental death and dismemberment (AD&D) insurance, as well as offering team members the opportunity to purchase additional life insurance, otherwise known as Voluntary Life Insurance. An added benefit to having Voluntary Life Insurance at Mazzella Companies is a service called WillPrep Services. WillPrep offers support and guidance to help team members prepare the documents needed to preserve their family’s security.

SurgeryPlus

SurgeryPlus, another supplemental benefit offered to plan participants and their dependents, is for non-emergency surgeries. Plan participants will have access to hundreds of surgical procedures performed by board certified surgeons including Spine, General Surgery, Genitourinary, Orthopedic, Ear, Nose & Throat, Cardiac, GI, and Pain Management. There will never be a need for a referral and Surgery Plus will handle all the billing so plan participants won’t receive any bills after the procedure.

When plan participants use SurgeryPlus, they will also have access to services such as travel assistance, a travel companion, and a surgery personal assistant who will serve as their patient advocate.

In addition to all the services provided, plan participant’s deductible and any coinsurance will be waived when a plan participant uses services through SurgeryPlus.

KIS Imaging

KIS Imaging is a supplemental benefit offering a concierge service when imaging work is needed such as an MRI, CT, or PET scan. Plan participants who are enrolled in Mazzella Companies’ healthcare plan will be able to reach out to the KIS Imaging scheduling service to help plan participants choose from a network of more than 2,600 nationwide radiology centers, schedule appointments, and handle paperwork.

Not only will plan participants’ deductible be waived, but benefits through KIS Imaging saves plan participants an average of 80% off the average imaging cost.

Free Drug Prescriptions

You read that right, FREE! Through our prescription provider, the only rule to receiving free drug prescriptions is that the plan participant must use generic versions of their prescribed drugs. If the providing pharmacy will only dispense the name-brand drug for any reason outside of the physician specifying the brand-name, then the plan participant will pay the difference between the generic and brand-name drug.

Ault International Medical Management (AIIM)

Providing information, education, resources, and support, AIMM will ensure that patients are getting the highest quality health care available. They act as a liaison between plan participants and medical professionals providing care ensuring that all prescribed medical care is “medically necessary and appropriate.”

All steps of the precertification process are handled by their skilled task masters including any non-emergency hospital, inpatient rehabilitation, and skilled nursing facility stays, transplant services, substance abuse and mental disorder treatment, hospice care, physical, speech, occupational, chemo, radiation, and cardiac therapy, dialysis, pre-natal and maternity care and any durable medical equipment costing over $500.


What Mental Health benefits does Mazzella Companies offer?

MyStrength

Mazzella Companies offers mental health care benefits through a Teladoc program called, MyStrength. MyStrength is a digital application with tools and resources to help guide you through creating a personal plan for your mental well-being.

There is no cost to schedule an appointment with a therapist or to use their self-guided coaching for depression, stress, sleep issues, personal concerns, and more. Plan participants enjoy convenient teletherapy services through the Teladoc app that include connecting with a licensed therapist by appointment seven days a week from the comfort of their home.

If your initial appointment with a myStrength therapist turns into future visits being scheduled, any costs associated with these future visits will be dictated by what health plan you choose – whether it is a Traditional Plan or HRA Plan.
